Membership and Billing Terms
These terms explain recurring billing, cancellation, refunds, and missed-appointment charges for the Medication Care membership.

Membership at a glance
The Medication Care membership is $89 per month after the separate $119 new patient visit. It includes ongoing physician-led medication management and follow-up as described on the Pricing section of the website. Medications and laboratory testing are purchased separately.
Billing date
Your first membership payment is charged on the date you enroll. The membership renews automatically on the same calendar date each month using the payment method on file. If a month does not contain that date, billing may occur on the last day of that month.
You are responsible for maintaining a valid payment method. If a payment cannot be processed, membership services may be paused until the account is brought current.
Cancel anytime
There is no long-term contract. You may cancel at any time before your next billing date by calling 602-638-1905 or sending a cancellation request through the secure patient messaging system.
Cancellation stops future renewals. Your membership remains active through the end of the billing period you have already paid for. A request received on or after a renewal has been processed applies to the following billing period.
Refunds
Membership payments and completed visit fees are nonrefundable and are not prorated for a partially used billing period. We will correct confirmed billing errors, and refunds will be provided when required by law.
Late cancellations and no-shows
Please provide at least 24 hours’ notice when changing or cancelling an appointment.
- Late cancellation: $35 when an appointment is cancelled or rescheduled with less than 24 hours’ notice.
- No-show: $50 when you do not attend and have not cancelled in advance.
These charges are separate from the monthly membership fee. They may be waived at the practice’s discretion for emergencies or circumstances beyond your control.
Clinical follow-up and prescriptions
Membership does not guarantee that a medication will be prescribed or continued. Prescriptions, refills, and dose changes are provided only when medically appropriate. Dr. Singh may require a completed follow-up visit, updated health information, vital signs, laboratory results, or an in-person evaluation before prescribing or renewing medication.
What membership is—and is not
The membership is a direct arrangement for the services described; it is not health insurance and does not replace comprehensive health insurance. It does not include emergency care, hospital care, specialist care, medications, laboratory testing, imaging, or services not specifically listed as included.
Changes or termination
We may update membership services, pricing, or these terms with advance notice when reasonably practicable. The practice may end or pause a membership for nonpayment, repeated missed appointments, safety concerns, abusive conduct, failure to follow essential monitoring requirements, or when continued care is no longer clinically appropriate. When applicable, we will provide information about transition of care.
