Telehealth Informed Consent
Please review this information before your first telehealth visit. You will have an opportunity to ask questions before agreeing to receive care through telehealth.

What is telehealth?
Telehealth means receiving care from Dr. Singh remotely, using secure video, audio, or other technology, instead of an in-person visit. It may include evaluation, diagnosis, treatment planning, and follow-up care.
Benefits
Telehealth lets you receive care without traveling to the office, at a time that may be easier to schedule, while still allowing Dr. Singh to review your history, discuss your goals, and manage your treatment plan.
Risks and limitations
As with any technology, telehealth has limits you should understand:
- The connection may fail, freeze, or disconnect during a visit.
- Dr. Singh cannot perform a hands-on physical examination. Some conditions may require an in-person visit, and she will tell you when that is needed.
- Information transmitted electronically carries some risk of interception or technical failure despite reasonable safeguards.
- Some medications, including controlled substances, may require a live audio-video or in-person evaluation and are subject to additional federal and state prescribing requirements. Availability through telehealth is not guaranteed.
- Telehealth is not appropriate for medical emergencies.
Your privacy
Your telehealth visit is confidential and protected in the same manner as an in-person visit. Records from telehealth visits become part of your medical record. We will not use images or information that identifies you for research or education without your separate authorization.
What we need from you
- A private, quiet location for your visit
- A stable internet connection and a device with a camera and microphone
- Your full attention during the visit, as you would give an in-person appointment
If the connection fails
If we lose connection during your visit, we will attempt to reconnect. If we cannot reconnect, we will call the phone number on file to finish the visit when clinically appropriate or to reschedule.
In an emergency
Telehealth is not for emergencies. If you are having a medical emergency, call 911 or go to the nearest emergency department.
Your rights
Telehealth is voluntary. You may choose an in-person visit instead, and you may withdraw your consent for future telehealth care at any time without affecting your right to future care. Withdrawing consent will not affect care already provided.
Consent
Before your first telehealth visit, you will be asked to confirm that you have read this page, understand the benefits, risks, and alternatives, and agree to receive care by telehealth. You may ask Dr. Singh questions before providing consent.
